Mental Health and Street Outreach Services

The Street Outreach team is a multi-disciplinary, multi-agency program that is centralized within Public Health, City of Hamilton’s Mental Health and Street Outreach Service. Outreach Workers work with individuals who are experiencing homelessness and have barriers to obtaining housing and supports within the community. St. Matthew’s House currently has one Street Outreach worker represented on the team.

The Outreach team assists individuals in connecting to supports around health care, income, housing and other appropriate community support services. Providing a presence on the street, in shelters, and other community drop-ins and programs, the Street Outreach worker reaches out to people who may not be able to get to St. Matthew’s House or other community partners to access supports. All team members work on building trusting relationships that assist individuals to connect or reconnect with community services and supports as needed.

To refer someone for outreach services, the individual must currently be experiencing homelessness and may or may not be experiencing issues relating to mental health.

Referrals are made by calling IntAc, Monday to Friday 8:30am – 4:30pm at 905-528-0683.
